Rule: Monitor

Please monitor your child/children at all times. Children are curious by nature. However, during the process of exploration, we would like to avoid any associated risk for injury. Never leave your child unattended in any area of the office.

Rule: Tidy Up

Please tidy up after yourself/family. We ask for your consideration of our office hygiene practices and respect for other families by simply cleaning up after yourself. Please avoid leaving trash or food behind in our waiting rooms, exam rooms, hallways and bathrooms. In addition, please do not vandalize or destroy any area or equipment within the office.

Cancellation Policy:

A $25.00 administrative charge will be applied to your account for missed appointments or appointments that are canceled with less than 24 hours notice.

HKP Family "House Rules"

These "house rules" are designed to promote a safe and healthy Environment for all families within the practice. We ask that you respect these rules and treat our office, staff, and other families the way you would like to be treated.

Rule: Smoking

Please avoid the use of any tobacco products (including e-cigarettes) within the confines of our office suite. While we understand it may be your preference to use these products, we have a duty to promote a healthy environment and image for all children within this practice.

Medical Assistant (MA) appointment:

Your appointment with the medical assistant is an integral part of your child's visit. This session allows our medical staff to complete all necessary vitals and data collection for your doctor, ensuring readiness for your appointment. In an effort to respect our other families and to ensure that you have enough time with your doctor, if you are not able to make your appointment with the MA we will need to reschedule your visit with the doctor.

Forms:

Please be advised that there will be a $25.00 charge for multiple page (5 or more pages) documents (such as school forms, Family and Medical Leave Act - FMLA forms) that need to be completed. Please allow 7 to 10 business days for completion. FMLA forms will only be completed for patients with ongoing/chronic medical conditions who have a well-established history with our clinic.

Cell Phones:

Please end all phone conversations when any provider from the Healthy Kids Pediatrics Team is addressing your child’s care. We plan on giving you our undivided attention during every encounter with your child. We ask that you extend the same courtesy to each member of our team.

Rule: Privacy

Please respect the privacy of other families within the office during the check-in and check-out process. We have a duty to uphold all Health Insurance Portability and Accountability Act (HIPAA) regulations when it comes to protected health information. So, we ask that you avoid crowding around the reception desk whenever there is a family receiving assistance from our staff.

Rule: Profanity

Please refrain from the use of profanity, for ANY reason. Young children acquire new language skills at a rapid rate. We would like to promote the growth of a healthy vocabulary. If there is an issue with any aspect of our practice, please bring it to our attention in a respectful and courteous manner.

Food:

Please do not eat or drink in the waiting areas or exam rooms.

No Shows:

Out of respect for our patients, if you have more than 3 no show appointments you may receive a letter of discharge from the practice.
